INSTRUCTIONS
FOR PATIENTS
PROVIDING ACCESS TO MY PATIENT CHART
To provide access to the Patient Chart:
- To a Physician/ Medical Staff - you must know your PESEL [Personal ID No] or ID (you can find it in your Patient Chart),
- To a Caregiver - you must know your ID number and have a mobile phone at hand with phone number that was used during the registration of your account. You will receive a text message with a code, provide this code to a Caregiver. This person must have an active Patient’s account in Istel Care system.
Providing access process starts the user (Physician/Medical Staff/Caregiver), who wants to get an access to your Patient Chart.
